The MAST (Multi-Agency Support Team) provides specialist and specific support to individuals who are frequently attending Emergency Departments. The aim of the project is to help reduce repeat admissions and release pressure over the winter period. The team is made up of 4 organisations, working collaboratively, from across the VCS; Project 6, The Cellar Trust, Age UK Bradford and Mind in Bradford, each focusing on a different specialist area. MAST is currently working in Airedale General Hospital and Bradford Royal Infirmary.

The team work with patients referred to them both in hospital and out in the community after discharge. 

The MAST aims to provide support around the following areas:

Frequent Attenders - Individuals who present regularly at Emergency Departments who require long term and sustained support. 

Enabling and supporting timely discharge & reducing risk - Individual who have shown increased admissions through Emergency Departments who with specialist support can reduce their hospital attendance. 

Reducing Admissions via health messaging - Speaking to first time patients in waiting rooms to ensure they aware of health messages that may prevent future admissions.
