Primarily we accept referrals from any Sheffield organisation supporting children and young people aged 5-18 who are:
- challenged by and experiencing poor mental health
- facing challenges connected to a disability
- at risk being excluded from school
- engaging in anti-social behaviour or criminal activity
- experiencing difficult family relationships
We work with MAST, Social Care, Pathways Team, schools, community youth teams and specialist children’s services.
To make a referral please complete the 3 parts of the referral form below. Please download, complete and return to firstname.lastname@example.org.
Young people can be referred to the project by any Sheffield agency that may feel that young people (5-19) would benefit from one to one mentoring support. We ask that the young person is made aware of the referral and understands what the mentoring services provides.
Please complete all areas on the referral forms which are relevant to the young person concerned. The more information we receive the better as this will help us find the right mentor for the young person.
We can only accept fully completed referrals this includes parents/carers consent.
