Sheffield Pathways

Sheffield Pathways provides support and advice for patients attending surgeries who are experiencing a social crisis. The team are able to offer specialist support over a 50-minute appointment to help individuals manage a range of non-medical issues, including:
• emotional wellbeing
• welfare advice
• housing
• domestic abuse
• substance use
Where longer-term support is required, Pathways are able to refer people into the right community service for their specific needs.
