How we can help

We provides specialist and specific support to individuals who are frequently attending Emergency Departments.

Working in partnership with The Cellar Trust, Project 6 and Carers Resource, each organisation focuses on a different specialist area providing support to frequent attenders, enabling and supporting timely discharge and reducing risk, and reducing admissions via health messaging.

Who this service is for

This service is for you if you are:

• a frequent attender at Airedale General Hospital or Bradford Royal Infirmary A&E
• referred to our service after discharge
